During surgery, the lids are held to the canthal tendons by the orbit. The lower lid can sag in an eyelid that has aged. A canthopexy procedure is changing the tilt of an eyelid, while a canthoplasty tightens the lower lid and brings the tissue under the eye tighter than it once was. In a canthopexy the bottom canthal tendon is tautened with a permanent suture sewed to the tissue under the skin. In a canthoplasty the tendon is split, relocated, and tightened. There are more surgical options to relocate the tendon in a canthoplasty than in a canthopexy. Over time the natural positioning of the eyelids adjust the location and gentle pressure of this repair and readjust to their new locations accordingly.
